Initially, dental adhesives evolved from no-etch systems to etch-and-rinse systems, which consisted of multiple components. From then on, manufacturers shifted focus toward simplifying the procedure. Different generations of self-etch adhesives and, finally multi-mode or universal adhesives were the result. KEY COMPONENTS OF UNIVERSAL ADHESIVES Universal adhesives are based on highly complex formulations with many different constituents and technologies combined in a single bottle. The most essential parts found in virtually every universal adhesive are: RESIN COMPONENTS Just like composite restorative materials, dental adhesives are resin-based materials. This means that different kinds of cross-linking and functional monomers are the beating heart of the formulation. Functional monomers are initially hydrophilic and form linear polymers upon curing, while cross linking polymers are hydrophobic and form stronger, highly cross-linked polymer networks.6 They are added to adhesive formulations to fulfil different tasks: some monomers are used to promote adhesion to tooth structure or composite resin, while others are used as etching or demineralizing agents, wetting enablers, and agents promoting penetration into the tooth structure. Hence, they have an impact on application properties, bond strength to various substrates including enamel and dentin, strength, hydrolytic stability and overall durability of the bonding layer, and more. INITIATORS To provide for a high strength of the adhesive layer and a favourable degree of conversion, dental adhesives are typically cured before the composite is applied. Universal adhesives contain a photo initiator that sets off the radical polymerisation reaction when light of a specific wavelength is absorbed. Chemical initiators are found in self- or dual-cure resin cements, but not in single-bottle adhesives. Studies have revealed that the initiator concentration has an impact on the mechanical properties of the materials they are part of 6,7. FILLERS Unlike composite restorative materials, which are always filled, dental adhesives may or may not contain fillers. Fillers are usually added for two main reasons: to increase the viscosity of the adhesive and to strengthen the adhesive layer. The viscosity has a direct impact on the thickness and quality of the adhesive layer. If the viscosity is too low, the risk increased of some areas of the tooth structure remaining uncovered resulting in an incomplete adhesive layer. In addition, an overly thin adhesive layer may be weak due to the inhibition of oxygen, which prevents proper curing of the contained resin6. If the viscosity is too high, however, application into the root canal and deep Class II cavities is often difficult thus increasing the risk of pooling or adhesive accumulation along margins. Moreover, air-blowing and complete solvent evaporation may also prove challenging. SILANE COUPLING AGENT A silane coupling agent is usually part of the formulation as well. They are the link between the resin matrix and the filler particles in the formulation and offer protection against degradation. SOLVENTS Solvents – typically water and ethanol or acetone – are essential parts of adhesive formulations. This is because dentin only bonds well to hydrophilic materials. Together with the hydrophilic monomers found in the formulation, the solvents provide for an environment favourable for dentin bonding6. Moreover, their task is to re-expand the collapsed collagen fibrils on the demineralized dentin surface. Most modern dental adhesives contain water and ethanol as solvents. However, ethanol cannot be used in combination with certain types of monomers. CHEMISTRY IMPROVEMENTS ENABLING THE ESSENTIAL BENEFITS There are certain components and technologies, like monomers and monomer combinations, which are used by various dental industry manufacturers in their universal adhesives. While some certainly offer advantages, potential drawbacks lie in their toxicity in the uncured state, possible unwanted interactions with other components and the like. RESIN TECHNOLOGY: QUICK AND STRAIGHTFORWARD APPLICATION PROCEDURE Adhesive penetration into the tooth structure is strongly influenced by monomers contained in the formulation. A monomer commonly used for this purpose is the highly hydrophilic hydroxyethylmethacrylate (HEMA)6. Although HEMA provides important benefits such as supporting homogeneous integration of components within the adhesive, its hydrophilic character – even after polymerization – may lead to water sorption, which can negatively influence the long-term mechanical properties of adhesive systems8. According to the results of an in-vitro study, high concentrations of HEMA in adhesive formulations may not only lead to poor mechanical properties (including bond strength), but also to a higher risk of bond degradation9. For these reasons, reducing the dependency on high levels of HEMA in universal adhesives and complementing it with alternative monomers is considered advantageous. However, this measure requires careful selection of the alternative monomer and precise alignment with the existing ingredients to prevent negative effects. In some cases, manufacturers have selected alternative monomers that lead to a lower bond strength or (unlike HEMA) lead to cytotoxic effects even after polymerisation10. In CLEARFIL Universal Bond Quick and the brand-new universal adhesive CLEARFIL Universal Bond Quick 2, Kuraray Noritake Dental Inc. have incorporated an amide monomer that significantly decreases the need for HEMA. In in-vitro tests, this monomer did not show any cytotoxic effects10, while it is responsible for one of the essential benefits – the extremely quick and straightforward application procedure. A popular strategy is the adding of cross-linking bi-or multi-functional monomers to the formulation6,12. While the most frequently used monomers added for this purpose are UDMA, Bis-GMA and TEGDMA, Kuraray Noritake Dental Inc. added the Urethane Tetra Methacrylate Monomer (UTMA) to add mechanical strength to the adhesive system. UTMA is a functional monomer that, due to its multiple curable sites, provides for an increased strength of the bonding layer. Both nano-hardness and ultimate tensile strength are extraordinarily high. Consequently, the adhesive offers the desired increased resistance to shrinkage forces. Moreover, it forms a highly curable, thin bonding layer. 3. FILLER TECHNOLOGY: MINIMISED RISK OF POOLING The new filler dispersion method also enables the formation of a thin, uniform layer independent of the cavity shape. The formation of an undesirably thick bonding layer along the margins and adhesive pooling at the bottom of the cavity are clearly a thing of the past. Hoe hebt u de behoefte aan een universal flowable restorative geïdentificeerd? Packables met een universeel kleurenconcept worden wereldwijd steeds populairder. Dat deed ons vermoeden dat er ook wel eens behoefte zou kunnen zijn aan een f lowable versie. Gebruikers profiteren bovendien van vereenvoudigde procedures en een minder aantal kleuren op voorraad met een positief effect op het voorraadbeheer als gevolg. Op basis van deze wetenschap hebben we de universal flowable in Japan geïntroduceerd waar het al een aanzienlijk marktaandeel heeft weten te veroveren. Gebruikers hebben het inmiddels ingezet voor verschillende doeleinden en uitstekende feedback geleverd. Kleurmatching bij anterieure en posterieure restauraties Waarom bieden jullie een extra kleur voor de anterieure regio? Composieten met universele kleuren moeten de kleur van de omringende tandstructuur aannemen om een perfecte match te garanderen voor een grote hoeveelheid doelkleuren. In het posterieure gebied zijn er meestal grote hoeveelheden omringende tandstructuur beschikbaar en zijn er nog meerdere caviteitswanden aanwezig. Dit maakt het aannemen van de tandkleur eenvoudiger ten opzichte van caviteiten in het anterieure gebied. In dat gebied schijnt het licht gemakkelijk door en wordt de kleur van de composiet zelf sneller weerkaatst. Dat kan leiden tot beperkingen in de kleurmatching. In het geval van CLEARFIL MAJESTY™ ES Flow Universal vertoonde de U kleur, die ideaal bleek voor het posterieure gebied, een aantal beperkingen ten aanzien van de kleurmatching van donkerdere tanden bij anterieure restauraties. Voor ons was dat voldoende reden speciaal voor deze situaties een tweede kleuroptie te bieden. Met U en UD biedt CLEARFIL MAJESTY™ ES Flow Universal een geweldige kleuraanpassing van lichte naar donkere kleuren om tegemoet te komen aan de behoefte aan precieze kleurmatching, met name in het anterieure gebied. Het ontwikkelen van slechts twee composietkleuren om aan te sluiten bij zoveel doelkleuren klinkt als een uitdaging. Hoe hebben jullie beoordeeld of de beschikbare kleuren geschikt zijn in de klinische omgeving? Het was inderdaad een behoorlijke uitdaging een U kleur te ontwikkelen die aan kan sluiten bij een breder scala aan tandkleuren voor zowel anterieure als posterieure tanden dan de U kleur van CLEARFIL MAJESTY™ ES-2 Universal. Om de door ons ontwikkelde kleuren te evalueren en verfijnen, hebben we herhaaldelijk vultests uitgevoerd met een verscheidenheid aan experimentele kleuren in kunstmatige tanden en getrokken tanden met verschillende tandkleuren. Bovendien hebben we onze uiteindelijke selectie van kleuren visueel laten beoordelen door tandartsen die de composiet gebruikten in de klinische omgeving. Wat zijn de grootste uitdagingen bij het bepalen en selecteren van de juiste kleur in de klinische omgeving? Het grote nadeel van de klassieke manier van kleurbepaling is dat het de behandeltijd onvermijdelijk vergroot. Voor zowel de tandarts als de patiënt is het minimaliseren van de behandeltijd echter een ultiem doel, omdat zo de kosten omlaag en het comfort van de patiënt omhoog kunnen. Het slechtste scenario in termen van tijd en comfort is een mismatch in kleur als gevolg van een fout tijdens de kleurbepaling. Dat kan intra-orale aanpassingen noodzakelijk maken of, in het ergste geval, leiden tot een nieuwe restauratie. CLEARFIL MAJESTY™ ES Flow Universal is verkrijgbaar in twee viscositeiten – Low en Super Low. Waarom is dat zo en wat is het verschil in gebruik? We hebben besloten twee typen vloeibaarheid met verschillende thixotropische eigenschappen aan te bieden om tegemoet te komen aan de verschillende voorkeuren van gebruikers in termen van verwerking. Ten aanzien van gebruik is er echter geen verschil tussen Low en Super Low. Gebruikers wordt echter aangeraden composieten met verschillende thixotropische eigenschappen te gebruiken op basis van hun voorkeuren en het specifieke geval. Zo is Super Low bijvoorbeeld geschikt voor het vormen van het occlusale oppervlak van posterieure tanden. Low kan dan weer alom worden gebruikt voor toepassingen als voeringen en vullingen in de cervicale regio en op proximale oppervlakken. CLEARFIL MAJESTY™ ES Flow Universal – Geweldige mechanische eigenschappen Wat is de technologie achter CLEARFIL MAJESTY™ ES Flow Universal waarmee een naadloze aansluiting mogelijk wordt gemaakt? Wat heeft u gedaan om de vorming van gaten tijdens het aanbrengen te voorkomen? Reflecting on this centenary, Satoshi Yamaguchi, president of Kuraray Noritake Dental, shares his perspective on how the company’s long-standing ethos continues to shape its direction. In this exclusive interview, he discusses Kuraray Noritake Dental’s latest solutions and the way the company combines scientific expertise with consistent quality, clear and evidence-based information, support grounded in everyday clinical needs, and a stable supply chain. Mr Yamaguchi, could you tell us about your professional background and how you first joined Kuraray? My Kuraray journey began with a strong aspiration to contribute to public health. I was drawn to the company’s extensive expertise in chemical manufacturing and the potential to apply its technologies in the medical field. I started in the medical development division, where I was involved in research and development activities. Later, I transitioned to a marketing role at Kuraray America and, upon returning to Japan, resumed my position in development. This allowed me to engage with the dental healthcare business from both a technical and a commercial perspective. Subsequently, I served as head of the dental business at Kuraray Europe in Germany, where I oversaw sales operations across the European region. These international experiences have profoundly shaped my professional approach, enabling me to lead with a global mindset and remain attuned to the specific needs of local markets. How has this international experience shaped your approach as president of Kuraray Noritake Dental? Through my international assignments, I came to recognise the importance of addressing the diverse values held across different countries and regions. In the field of dental materials, preferences often differ. In many parts of Europe and Asia, natural aesthetics are widely valued; in the US, treatment efficiency is often prioritised; and in South America, there is often an emphasis on balancing aesthetics with cost-effectiveness. Although these are general tendencies and can vary, they provide a useful guide in meeting the specific needs of each market. Satoshi Yamaguchi, president of Kuraray Noritake Dental, previously served as head of Kuraray Europe’s dental business, based in Germany. My goal is to establish a robust framework for delivering reliable, evidence-based information to dental professionals. We are striving to strengthen the foundation of trust by facilitating the collection of credible data and case studies from both internal and external sources, ensuring that the information we provide contributes to greater confidence in clinical practice. This means versatile, high-performance solutions that streamline clinical workflows by minimising the number of required components and procedural steps. What sets your proprietary zirconia powder and multilayer technology apart in today’s competitive CAD/CAM market? We consistently develop and manage raw material powders in-house. By focusing on proprietary material development, we have created high-quality dental zirconia that combines aesthetics and mechanical strength. In addition, our advanced manufacturing technologies and strict quality control standards ensure stable and reliable product quality. Furthermore, by utilising our proprietary zirconia powder and working closely with manufacturers of milling machines and sintering furnaces, we have significantly reduced both processing and sintering times in the fabrication of zirconia restorations. One notable example is KATANA Zirconia ONE SPEED CROWN, which supports a streamlined chairside workflow. We have developed a new zirconia powder that can reach a highly activated state with little energy. This accelerates the sintering process, thereby achieving an ultra-short sintering time of just 9 minutes. Furthermore, our zirconia and multilayer technology enable natural gradation by varying both shade and translucency across the layers of the zirconia disc—closely replicating the appearance of natural teeth. Leading up to this year, we significantly strengthened our global supply system for dental materials. At the heart of this initiative was the strategic expansion of production capacity at our Miyoshi plant, where we ultimately aim to roughly double production capacity. This facility specialises in the manufacturing of inorganic dental materials such as zirconia and porcelain and plays a critical role in our business strategy through technological expertise, rigorous quality control and product development capabilities. Additionally, by strengthening collaboration between our manufacturing site and our research and development, we are working to establish a system that enables rapid incorporation of feedback from clinical settings into product improvements. These efforts also contribute to enhancing our product development capabilities, allowing us to deliver dental material products that meet market needs in a timely manner. Initially, dental adhesives evolved from no-etch systems to etch-and-rinse systems, which consisted of multiple components. From then on, manufacturers shifted focus toward simplifying the procedure. Different generations of self-etch adhesives and, finally multi-mode or universal adhesives were the result. The latter category, introduced in the early 2010s, nowadays comprises numerous products offered by many different manufacturers. In general, universal adhesives offer more procedural freedom and simplicity than multi-step, multi-bottle systems as well as good adhesion. According to a recent review1, adhesive properties have improved and manufacturers have also succeeded in reducing the technique sensitivity of the adhesive systems. However, hybrid layer degradation still seems to be a relevant issue for many modern products available on the market. Moreover, feedback from clinicians reveals that there are several areas of concern where bonding systems in general can be improved. These include the risk of contamination due to a lengthy procedure or the need for rubbing, pooling of the adhesive in corners and accumulation along margins. Moreover, some adhesives are challenging to introduce into narrow cavities, while their curability may be limited.
