Procédure simplifiée de restauration postérieure sans sélection de couleur

L'efficacité est un facteur important quand il s'agit de restauration postérieure de cavités avec une résine composite. L'utilisation d'un matériau qui élimine la nécessité de sélectionner la couleur et l'application de techniques de stratification simplifiées, par exemple, permettront d'atteindre cette efficacité. Dans Ie cas clinique ei-dessous, une première molaire et une seconde prémolaire maxillaires ont été restaurées avec CLEARFIL MAJESTY" ES-2 Universal (U) et la technique de la couleur unique.

Situation initiale. Des lésions carieuses sont détectées sur les surfaces occlusale et palatale de la première molaire et dans l'aspect distal de la seconde prémolaire. Une digue en caoutchouc a été placée pour contröler l'humidité.

Aspect des dents après retrait des caries, mordançage sélectif de l'émail et application de CLEARFIL SE BOND 2.

Matrice sectionnelle en place. Elle est utilisée pour faciliter l'établissement d'un contact interproximal adéquat.

Dental photography as a valuable documentation aid

The main purpose of photography in dentistry is documentation. The aim is to collect as much information as possible under reproducible conditions. For this purpose, the camera used, its accessories and the format and lighting of the intra- and extra oral recordings relevant to dentistry must be standardized. Under standardized photographic conditions, the recordings made can be compared with one another, even if the later recordings are made only after a long period of time and by different photographers. Only in this form can dental photography be used as a valuable documentation aid. The standards described by Wolfgang Bengel in 1985 are still valid, but they have to be updated and adapted to technical innovations.

In this webinar, the current development in the field of dental photography and related areas will be briefly explained.

Zirconia, Oral scanner, feather edge preparation: the “winning triad”

The introduction of digital technology associated with the development of the latest zirconia materials offers the clinical and the dental technician a wide range of solutions to achieve aesthetics and function in a simple, predictable and above all conservative way. A further type of vertical preparation called "Minimally Invasive Vertical Preparation" has been introduced, applicable to numerous prosthetic clinical situations including cases of aesthetics. Minimally invasive vertical preparations, new type of zirconia and intraoral scanners represent a “winner triad” for today's prosthetic therapy.

Graduated with honors in Dentistry at the University of Siena in 1992, wins the scholarship of the Italian Society of Periodontology (SIDP) for the year 1994-95. He therefore has the opportunity to attend the Department of Periodontology and Fixed Prosthesis of Prof. NP. Lang at the University of Bern in Switzerland, carrying out clinical and research activities. He obtained the title of "Doctor Medicinae Dentium" (D.M.D.) at the same University in 2000.

Author of scientific publications in international journals, he is an Active Member  of the Italian Academy of Prosthetic Dentistry (AIOP). En dehors du mordançage du tissu dentaire, l’acide phosphorique est également beaucoup utilisé en tant que produit de nettoyage intra-oral et extra-oral. Cependant, l’acide phosphorique est-il toujours le produit adapté ?

Des restaurations directes

De nombreuses procédures de collage sont effectuées selon le principe de l’auto-mordançage. Les monomères acides du primer et/ou de l’adhésif modifient la couche de lubrification et corrodent légèrement la surface de la dent. Il en résulte une adhésion chimique et mécanique entre l’adhésif et le tissu dentaire.

En cas de contamination dans la cavité, il n’est pas souhaitable de l'éliminer avec de l’acide phosphorique. En effet, l’hydroxyapatite est alors éliminée par mordançage de la surface de la dent, empêchant toute adhésion chimique.

Katana Cleaner contient un sel MDP de triéthanolamine, abrégé MDP-TEA, ainsi que du MDP « libre ». En raison du caractère quasiment neutre (pH 4,5) de KC, aucun HAp ne sera éliminé, contrairement à ce qui se produirait en cas d’utilisation d’acide phosphorique. Cependant, toute contamination éventuelle par de la salive et/ou du sang est éliminée efficacement.

Dispositifs indirects

Pour les matériaux contenant du verre comme le disilicate de lithium et la porcelaine feldspathique, l’acide phosphorique est un moyen éprouvé pour éliminer les salissures, par ex. le dépôt résultant du mordançage avec HF.  Cependant, une utilisation routinière de l’acide phosphorique pour débarrasser les dispositifs indirects d’une contamination peut ne pas s’avérer judicieuse. Ainsi, son utilisation sur l’oxyde de zirconium est même déconseillée. Il influence négativement l’adhésion chimique de monomères phosphatés à la surface du zirconium.

Lors du cimentage de dispositifs prothétiques avec des ciments auto-adhésifs (par ex. PSAU) ou des ciments utilisant des primer auto-mordançants (Panavia f2.0 ou V5), l’utilisation d’acide phosphorique sur la dentine n’est pas indiquée. Et ce, pour les mêmes raisons qu’en cas de restaurations directes.

Contrairement à l’acide phosphorique à 35%, Katana Cleaner est un produit qui ne présente pas de risque. Vous pouvez également vous référer à mon précédent blog sur l’acide phosphorique.
