Mathias Fernandez Lombardi, wie zouden moeten overwegen om te werken met KATANA™ Zirconia YML?

“KATANA™ Zirconia YML is ontworpen voor laboratoria en freesinrichtingen, waarbij vereenvoudiging centraal stond. Dankzij de sterke body en hoogtranslucente
glazuurlaag biedt dit product precies wat nodig is voor een onbeperkt indicatiespectrum. KATANA™ Zirconia YML is bij uitstek geschikt voor de vervaardiging van duurzame monolithische posterieure longspan-bruggen en hoog esthetische anterieure kronen of gedeeltelijke restauraties. Daarmee is dit product ongekend veelzijdig en is er geen behoefte meer aan ander zirkonium- of zelfs keramisch CAD/CAM-materiaal. Bovendien ondersteunt het hooggeautomatiseerde

Wat zijn de voordelen van het gebruik van één materiaal voor elke indicatie?

“Er zijn verschillende voordelen. Zo hoeven er minder blanks te worden opgeslagen. Dat vereenvoudigt het voorraadbeheer en minimaliseert de vereiste opslagruimte. Daarnaast bevordert het de standaardisering van workflows in het laboratorium. Consistent gebruik van hetzelfde materiaal betekent dat er geen verschillen zijn in het basisontwerp en de freesparameters - zoals minimale wanddikte en sterkte van de verbinding - in de sinterprotocollen en de opties voor afwerking waarmee rekening moet worden gehouden. Er worden dus minder snel fouten gemaakt en de routines zijn gemakkelijk te hanteren. Tot slot wordt het risico op kruisbesmetting uitgesloten als er één materiaal wordt verwerkt met de beschikbare apparatuur (freesapparaat, frees- en afwerkingsinstrumenten en sinteroven). Dat betekent: voorspelbare uitkomsten en een onberispelijke esthetiek.”

Wat zijn de voordelen van KATANA™ Zirconia YML als uniek keramisch CAD/CAM-materiaal?

“Het belangrijkste argument is volgens mij dat esthetische resultaten gemakkelijk en efficiënt worden bereikt voor allerlei restauraties, zelfs voor longspan-bruggen. Het is doorgaans voldoende om monolithische restauraties te vervaardigen voor het posterieure gebied, die dan gewoon worden gepolijst of geglazed. Anterieure restauraties kunnen monolithisch of met minimale cut-back worden ontworpen. De gebruiker kan kiezen tussen CERABIEN™ ZR (CZR) FC
Paste Stains en CZR Internal Stains plus glaze om een natuurlijke uitstraling te bereiken. Nóg een voordeel van KATANA™ Zirconia YML is de naadloze multilayer-structuur, in combinatie met een uitzonderlijk hoge kwaliteit van de blank.”

Waarom zijn de naadloze structuur en hoogwaardige kwaliteit van de blank belangrijk voor gebruikers?

“De naadloze multilayer-structuur, ofwel een soepele overloop tussen de ene laag en de andere, is een absolute voorwaarde voor structurele integriteit en onberispelijke esthetiek. Dankzij deze structuur is het materiaal vrij van transitielijnen en staat het vanwege de uniforme krimp garant voor precieze maten
na het sinteren. Bovendien functioneert het ook op lange termijn betrouwbaar, vrij van interne spanning. Dat komt door de volledig homogene samenstelling, met zorgvuldig afgestemde CTE-waarden en krimpverhoudingen in alle delen van de blank.”

“ Met KATANA™ Zirconia YML worden gemakkelijk en efficiënt esthetische resultaten bereikt voor allerlei restauraties, zelfs voor longspan-bruggen.”

“Voor de ontwikkeling van dit product heeft Kuraray Noritake Dental haar in jaren opgebouwde kennis van tandheelkundig keramiek ingezet, samen met de voordelen van de volledige productie van blanks in eigen bedrijf. Anders zou het onmogelijk zijn geweest om het gevraagde kwaliteitsniveau te bereiken. Aangezien dit materiaal zo goed is afgestemd, was het zelfs mogelijk om een sinterprogramma van 54 minuten te ontwikkelen dat net zulke goede resultaten oplevert als de beproefde programma's van 90 minuten of 7 uur. Hiermee is dit dé optie voor alle spoedgevallen. Dankzij die eigenschap is KATANA™ Zirconia YML nu het meest veelzijdige materiaal: het biedt comfort en veiligheid op het niveau van een SUV, maar als het op snelheid aankomt, doet het niet onder voor
een sportauto.”

Bovenstaande sinteradviezen zijn slechts een richtlijn. Afhankelijk van de individuele oven en de omstandigheden kan bijstelling nodig zijn.
Als het sinterprogramma voor 54 of 90 minuten niet programmeerbaar is in uw oven, is het niet mogelijk om de oven volgens een van deze schema's in te stellen.
*Het materiaal wordt bij 800°C uit de oven verwijderd. Er is een oven nodig met een configureerbaar YML-sinterprogramma.

Gebruikers die al keramische materialen met sterkteverloop hebben getest, weten dat de positionering van longspan-restauraties in de blank
lastig kan zijn. Met welke regels moet je rekening houden bij de positionering van een longspan-brug in een YML-blank?

“De plaatsing van restauraties in KATANA™ Zirconia YML discs is bijzonder gemakkelijk. Dat komt doordat de kloof tussen de laagste buigsterkte in het glazuurgebied en de hoogste buigsterkte in de onderste bodylaag naar verhouding klein is. Daarbij komt nog dat bodylaag 1, die naast de glazuurlaag ligt, al een buigsterkte biedt die hoger is dan de 800 MPa die is vereist voor bruggen met vier of meer elementen. Gebruikers bevinden zich dus aan de veilige
kant als ze hun longspan-restauraties in het midden van de blank positioneren. Meer specifiek dient de helft van de overdwarse connector zich in de onderste helft van de disc te bevinden. Dat is sterk afwijkend van andere materialen met sterkteverloop, waarbij de verbinder uitsluitend in het bodygebied mag worden gepositioneerd. KATANA™ Zirconia YML restauraties met maximaal drie elementen mogen in elke laag worden gepositioneerd. Het feit dat de dikte van
elke laag proportioneel toeneemt met de hoogte van de disc, levert nog een voordeel op. De glazuurlaag is namelijk ruim genoeg om haar esthetische potentieel te benutten bij enkele elementen of kleine bruggen.”

Wat zou u aanbevelen aan tandtechnici die de slagkracht van hun laboratorium of freesinrichting willen vergroten?

“Ik kan echt aanraden om KATANA™ Zirconia YML te testen in een laboratoriumomgeving om te zien hoe het werkt en de mogelijkheden te ervaren. De uitzonderlijke kwaliteit van de blank en naadloze integratie van de verschillende lagen, het gemak van positionering en de efficiency bij de verwerking moeten potentiële gebruikers echt in de praktijk ervaren om te begrijpen wat het betekent voor het dagelijks werk. Op diezelfde manier moeten de esthetische resultaten binnen de kliniek worden bekeken om een goede indruk te verkrijgen van de uiteindelijke natuurlijke look & feel. Wil je de esthetiek van keramisch materiaal verbeteren - vooral de translucentie - dan gaat dat vaak ten koste van de buigsterkte. Bij het nieuwe KATANA™
Zirconia YML van Kuraray Noritake Dental Inc. ligt dat anders. De onderste helft van de blank heeft een hoge buigsterkte van 1100 MPa, terwijl het bovendeel van de body en de incisale gebieden een hoge translucentie hebben. Dankzij dit nieuwe type multi-layered zirkoniumoxide kunnen zelfs esthetische monolithische restauraties worden vervaardigd die geschikt zijn voor het anterieure gebied. Het product geeft een hoge mate van ontwerpflexibiliteit. De hoge translucentie in het incisale gebied zorgt voor een natuurlijke uitstraling na het sinteren. De ultra-microlayering en het glazen op het monolithische oppervlak geeft een natuurlijk resultaat, waar patiënten blij mee zijn.

It is strong, it is pre-shaded like natural teeth, and it is reliable: KATANA™ Zirconia HTML, the highest strength version among KATANA™ multi-layered zirconia series. Now, Kuraray Noritake Dental Inc. has carefully optimized the product to further improve its optical properties and to harmonize the sintering schedules with all other materials of the KATANA™ Zirconia Multi-Layered Series.

The new KATANA™ Zirconia HTML Plus offers all popular properties of its predecessor, including the multi-layered colour structure and uniform strength distribution throughout the blank. It is suitable for a variety of indications including long-span bridges with a minimum wall thickness of just 0.4 mm in the anterior and 0.5 mm in the posterior region. This allows for minimally invasive preparations, while the user may choose between a monolithic, cut-back or framework design combined with the preferred finishing technique.

The PLUS refers to additional benefits like the material’s improved optical properties: KATANA™ Zirconia HTML Plus offers an increased translucency of 45 percent (compared to 43 percent KATANA™ Zirconia HTML) and an even more natural shade appearance of the manufactured restorations. “As the colour of the restorations appears deeper, more vivid and more saturated, and the translucency is closer to that of natural teeth, a great optical integration is achieved straight away. This leads to a reduced finishing effort”, says Mathias Fernandez Y Lombardi, EU Scientific Manager Dental Ceramics & CAD/CAM Materials at Kuraray Noritake Dental. At the same time, the high flexural strength (1,150 MPa) is maintained, providing for an unlimited range of indications.

Like its predecessor and all other materials of the KATANA™ Zirconia Multi-Layered Series, KATANA™ Zirconia HTML Plus is the result of an in-house development, powder compilation and blank production process that is able to control every detail. This enabled our specialists to achieve a precise alignment of the desired properties, while maintaining a high blank quality that leads to the accurate milling results with smooth surfaces and regular edges users of KATANA™ Zirconia HTML are familiar with.

The unique powder composition and specific in-house blank production procedure also allowed us to streamline the sintering protocols. For single-tooth restorations and bridges with up to three units, three different protocols are available: a 54-minute high-speed sintering programme specifically developed for remake and rush cases, a 90-minute schedule and the traditional seven-hour sintering programme. The latter is also suitable for long-span restorations.

These three options and identical schedules are available for KATANA™ Zirconia HTML Plus, KATANA™ Zirconia UTML, KATANA™ Zirconia STML and the new KATANA™ Zirconia YML with additional flexural strength and technical translucency gradation. With this feature, it becomes extraordinarily easy to standardize procedures despite an indication-specific selection of the best-suited KATANA™ Multi-Layered Zirconia.

We had a conversation with three of them about the specific features of KATANA™ Zirconia YML and its potential role in the dental practice and laboratory. Mathias Fernandez Y Lombardi, EU Scientific Manager Dental Ceramics & CAD/CAM Materials, provided insights from inside the company, while Prof. Dr. Florian Beuer MME, Charité – Universitätsmedizin Berlin and MDT Nondas Vlachopoulos, owner of the dental laboratory AestheticLab in Athens, shared their external perspectives.

What are the most important features and benefits of the new KATANA™ Zirconia YML?

Mathias Fernandez Y Lombardi: KATANA™ Zirconia YML is a 5^th generation zirconia featuring next-generation multi-layered technology: it offers colour, translucency and flexural strength gradation in one disc – for a virtually unlimited indication range. As it may be used as the only zirconia or even ceramic CAD/CAM material in a laboratory or milling centre, it is designed for everyone who would like to simplify and standardize laboratory procedures. It may be sintered within 54 minutes, offers a high accuracy of fit and may be finished using time-saving techniques.

What are the benefits of using a single zirconia for every indication from the dental technician’s perspective?

Nondas Vlachopoulos: The use of a universal zirconia solution with a high aesthetic potential and well-aligned mechanical properties allows dental technicians to reduce their inventory and streamline their procedures – from designing to finishing. Nowadays, we all know that, in order to optimize the outcomes, it is essential that we develop a deep understanding of a material’s properties, respect its design parameters (like minimum wall thicknesses) and adhere to the sintering protocols. It is much easier to accomplish this for a single material than for many different ceramics, and the potential for errors is minimized with a single-material approach. With KATANA™ Zirconia YML, this approach enables us to produce very different outcomes depending on where the restoration is placed in the multi-layered blank.

What are the benefits of using KATANA™ Zirconia YML as the only ceramic material from the dental practitioner’s perspective?

Florian Beuer: To my mind, the most important benefit of using a single zirconia every time lies in a simplified documentation. When different ceramic materials are used in the dental laboratory, we need to keep track of the material selected in every single case. Only in this way, it is possible to make the right cementation decisions (regarding cement selection and the appropriate pre-treatment of the restoration). In addition, when it comes to failure of the restoration, we need to know which material was used to prove that it has been processed correctly and is suitable for the actual indication. This is much easier when a single material is used every time. Zirconia is predestined for this approach as it is forgiving, supports minimally invasive preparations and – in case of KATANA™ Zirconia YML – almost everything can be done with it.

What are the secrets of KATANA™ Zirconia YML that allow for excellent treatment outcomes despite the one-for-all approach?

Mathias Fernandez Y Lombardi: KATANA™ Zirconia YML is based on a completely new, seamless multi-layer technology combining different, newly developed raw materials with varying yttria concentrations in a single blank. The inhouse development of the powders and their precise adjustment of each other in terms of properties like shrinkage and coefficients of thermal expansion are prerequisites for a smooth transition from one layer to the next. Only if the layers are very well integrated, it will be possible to reliably produce even long-span restorations with a delicate design, perfect fit and flawless aesthetics (without transition lines). The aesthetic properties were also optimized by integrating clinical instead of purely technical translucency.

Please explain the term clinical translucency.

Mathias Fernandez Y Lombardi: Usually, translucency of dental materials like zirconia is measured in its white, pre-shaded state. This so-called technical translucency value is increased by adding yttria to the formulation, which makes the zirconia itself appear brighter (increasing the whiteness). The translucency of the final restoration, however, is also affected by pigments and other additives in the formulation. Consequently, monolithic restorations made of different materials with the same technical translucency value may vary widely in terms of their actual, clinical translucency, which affect the overall aesthetic potential. The formulation of KATANA™ Zirconia YML has been optimized by carefully selecting and combining the right additives to obtain a natural, tooth-like translucency.

Nondas Vlachopoulos, is this measure of optimizing the clinical translucency a key feature enabling the one-for-all approach of KATANA™ Zirconia YML?

Nondas Vlachopoulos: The positive effect attributed to this measure is noticeable, as KATANA™ Zirconia YML has a particularly warm and natural appearance especially in the body area some other materials with flexural strength gradation are lacking. This leads to an exact colour match with the VITA classical A1-D4 shade guide, and reduces the need for manual shade adjustments. This is important for a one-for-all approach, as the material should be a perfect basis for the production of monolithic restorations, which are not covered by a thick porcelain layer able to mask any imperfections. However, there is another key feature that facilitates the zirconia’s use for virtually every indication: Its exceptional design flexibility. It enables users to control the translucency and overall aesthetics by positioning the restoration quite freely within the blank. For restorations with up to three units, there is a complete freedom of placement. Larger restorations with four or more units need to be positioned in a way that at least 50% of the connector cross-section is in the bottom half of the disc. This is easily accomplished and gives us optimal options to exploit the various properties of each layer depending on the aesthetics and strength needed in each specific case.

Positioning guidelines for KATANA™ Zirconia YML.

What is the recommended finishing procedure?

Mathias Fernandez Y Lombardi: In principle, all designs and finishing options are available for KATANA™ Zirconia YML – from a monolithic design individualized with liquid ceramics to microlayering after a minimal vestibular cutback or even full ceramic layering on frameworks. As the aesthetic potential of the zirconia is already high, however, we believe that in most cases, the first option is absolutely sufficient. In this case, a monolithic restoration is milled and the surface subsequently treated with our liquid ceramic system CERABIEN™ ZR FC Paste Stain. The pastes are easily applied in ultra-thin layers, very well controllable and wear resistant.

Nondas Vlachopoulos, what is your preferred finishing procedure for restorations made of KATANA™ Zirconia YML?

Nondas Vlachopoulos: In the posterior area, I often opt for monolithic restorations individualized with CERABIEN™ ZR FC Paste Stain. The technique leads to beautiful outcomes a large majority of patients would love to see even in the anterior region. However, as orders for the production of anterior restorations coming my way are usually placed by clinicians and patients with the highest possible aesthetic demands, I often go for a framework design and full porcelain layering with CERABIEN ZR Porcelains. The materials (like EX-3 portfolio for metal frameworks) are based on synthetic feldspathic porcelain with a structure and particle distribution very similar to natural dentin and enamel. All in all, I like the fact that I can select the most suitable finishing technique for every individual case, and I believe that it is often possible to reduce the finishing effort to a minimum.

Clinical translucency, design flexibility, mechanical properties: Apparently, the differences between the available zirconia materials – even of the 5^th generation – are huge. How can users ensure that the material they select is of high quality and offers favourable mechanical as well as optical properties?

Florian Beuer: When the first-generation zirconia materials appeared on the dental market, laboratory testing revealed that there are huge differences in quality between materials of different manufacturers. Although independent studies comparing the properties and behaviour of 5^th generation zirconia are still scarce, it may be assumed that differences do exist. My recommendation is to select a several materials from well-known manufacturers supporting clinical studies, compare the study results already available, and test the selected options in the laboratory setting. Directly comparing design concepts, the processing quality and final appearance will most likely reveal which material is a dental technician’s favourite. I am sure that KATANA™ Zirconia YML is among the materials worth being tested.

Het Amerikaanse Dental Advisor is een instelling die klinische praktijkevaluaties en producttesten uitvoert. Eenmaal per jaar worden de onderscheidingen ‘Top Product’ of ‘Preferred Product’ toegekend aan de tandheelkundige materialen en apparatuur met de beste prestaties. Daarnaast geeft het bedrijf Editors’ Choice Awards uit, voor unieke, belangrijke producten.

Top Product Awards

CLEARFIL™ SE ProtectCategorie Directe Restauratieven, Hechtmiddel: Zelfetsend.

Dit zelfetsende adhesief is gebaseerd op de samenstelling van de Gouden Standaard tweestaps zelfetsende adhesief CLEARFIL™ SE BOND. SE Protect bevat echter een extra MDPB-monomeer, dat een antibacterieel reinigingseffect van de caviteit en langdurige afgifte van fluoride geeft.

De onderscheiding van Dental Advisor is gebaseerd op een materiaaltest die gedurende 6 weken is uitgevoerd in de praktijk van 13 adviseurs en redacteuren van Dental Advisor. Aan het antibacteriële zelfetsende adhesief werd, vanwege de uitstekende prestaties, een klinische score van 98% toegekend.

CLEARFIL MAJESTY™ ES Flow.

Categorie Directe Restauratieven, Composiet: Hooggevulde Flowable.

Voor het zesde jaar op rij kreeg CLEARFIL MAJESTY ES Flow deze onderscheiding. Deze universele vloeibare composiet wordt geleverd in drie niveaus van vloeibaarheid, van High tot Super Low. Op deze manier kan de gebruiker de juiste viscositeit kiezen voor elke klinische situatie. Daarbij is de hoogvloeibare (High) variant de perfecte keuze voor caviteits-liner en als coating in IDS-procedures. De variant met een superlage vloeibaarheid (Super Low)  wordt specifiek gebruikt voor directe veneerprocedures en zelfs voor de opbouw van posterieure cusps etcetera.

De universele vloeibare composiet werd binnen meer dan 900 klinische toepassingen getest door 29 adviseurs van Dental Advisor. De onderzochte eigenschappen - aanbrengen/verwerking, esthetiek, viscositeit en polijstbaarheid - werden beoordeeld als 'uitstekend'. Dit leidde tot een aanbevelingspercentage van 100% en een algehele klinische score van 98%. Overigens heeft Dental Advisor alleen het standaardniveau van vloeibaarheid (Low) beoordeeld, aangezien de twee andere varianten niet verkrijgbaar zijn in de VS.

TEETHMATE™ DESENSITIZER

Dit product won al zes keer eerder een prijs van Dental Advisor. TEETHMATE DESENSITIZER behandelt overgevoeligheid, door hydroxyapatiet (HAp). Het hydroxyapatiet wordt precies daar gevormd waar het nodig is om de dentinetubuli en barstjes in het glazuur af te sluiten. TEETHMATE DESENSITIZER geen negatief effect op de hechtsterkte van tandheelkundige adhesieven of cementproducten.

Uit een klinische evaluatie van 6 maanden, uitgevoerd door Dental Advisor bij 27 patiënten met overgevoeligheid door terugtrekkend tandvlees kwam naar voren dat het gebruik van het product zeer effectief is, aangezien het onmiddellijk en zelfs op lange termijn (6 maanden) uitkomst biedt bij overgevoeligheid. Dit resulteerde in een uitstekende beoordeling met een score van 96%.

PANAVIA™ SA Cement Plus

Categorie Zelfhechtend Cement. Dit dual-cure zelfhechtende composietcement onderscheidt zich door het gebruiksgemak, de gunstige viscositeit en gemakkelijke verwijdering van overmaat na tack-curing.

Preferred Products

PANAVIA™ SA Cement Universal.

Dit zelfhechtende universele composietcement uit 2020 bevat het unieke LCSi-monomeer. Het unieke silaan - LCSi-monomeer – in PANAVIA™ SA CEMENT Universal zorgt voor een directe, sterke en duurzame chemische hechting aan vrijwel alle materialen: porselein, lithiumdisilicaat, composiet en zelfs glaskeramiek. Bovendien zorgt de hoge concentratie van het originele MDP-monomeer voor de chemische reactiviteit met zirkoniumoxide, metaallegeringen, dentine en glazuur. 

Gezamenlijk zorgen deze technologieën ervoor dat PANAVIA SA Cement Universal een sterke en duurzame hechting aangaat met vrijwel elk restauratief materiaal. Een afzonderlijke primer of voorbehandeling is niet meer nodig.

 31 klinische beoordelaars van Dental Advisor, die het cement hadden gebruikt in 516 applicaties, betitelden het product als ‘heel goed’ tot ‘uitstekend’ vanwege de verwerkingseigenschappen, esthetiek en tandvleesvriendelijkheid. Het resultaat van de algehele klinische beoordeling lag op 96%.

CLEARFIL™ Universal Bond Quick 

Voor het tweede jaar op rij werd CLEARFIL Universal Bond Quick door Dental Advisor onderscheiden als Preferred Product. Dit universele adhesief kan worden gebruikt met een zelfetstechniek of - samen met K-ETCHANT Syringe  (fosforzuur)- binnen de selectieve techniek van glazuurets of total-etch.

Dit adhesief bevat onze Rapid Bond-technologie: een combinatie van origineel MDP-monomeer met innovatieve hydrofiele amidemonomeren. Het werkt direct; wachten, meerdere lagen of uitgebreid inwrijven is niet meer nodig.

De adviseurs van Dental Advisor gaven CLEARFIL Universal Bond Quick een score van 98% in een klinische evaluatie.

CLEARFIL™ CERAMIC PRIMER PLUS

Voor de derde keer op rij werd CLEARFIL CERAMIC PRIMER Preferred Product. Het product bevat het originele MDP-monomeer en een silaanmonomeer, die garant staan voor een uitstekende hechtsterkte aan uiteenlopende soorten restauratieve materialen: keramiek op silicabasis, zirkonium, composieten en zelfs metalen. Aanbrengen is heel simpel; breng Clearfil Ceramics Primer aan na de aanbevolen voorbehandeling en droog het hechtoppervlak.

Bij een klinische evaluatie kenden de adviseurs van Dental Advisor een aanbevelingsscore van 96% aan het product toe, evenals een algehele score van 96%.

Conclusies

Er zijn verschillende manieren om onderzoek uit te voeren. Zo biedt laboratoriumonderzoek informatie over specifieke producteigenschappen. Dit type onderzoek staat echter meestal niet garant voor een totaalbeeld. Bovendien zijn de verschillende testresultaten nauwelijks vergelijkbaar vanwege de uiteenlopende testomstandigheden.

Klinische testresultaten zijn erg nuttig, omdat ze licht werpen op de klinische prestaties. Nadeel is wel dat deze prestaties meestal pas jaren na een productintroductie worden gepubliceerd.

De manier waarop Dental Advisor producten beoordeelt, met klinische praktijkevaluaties, geeft waardevolle informatie. Potentiële gebruikers kunnen hierop terugvallen om hoogwaardige materialen te selecteren.
