Their opinion: “The material itself has very nice handling properties, allowing easy seating of veneers with virtually no rebound. It is also not runny (flow is 5), but at the same time, it is not overly thick that you risk fracturing an ultrathin veneer. In addition, its consistency facilitates loading a veneer and cleaning the excess with minimal to no stickiness.”

CLEARFIL™ SE Protect: A uniquely antibacterial adhesive system

In the early 1990s, Kuraray introduced CLEARFIL™ LINER BOND 2, the first self-etching adhesive, to the world. Among the products that this system has led to is the award-winning CLEARFIL™ SE Protect, which was introduced in 2004 and remains highly valued by dental professionals across the world. By combining Kuraray Noritake Dental’s famed original MDP monomer with the antibacterial 12-methacryloyloxydodecylpyridinium bromide (MDPB) monomer, this self-etching adhesive is able to kill bacteria on contact. How is this possible? Let’s find out.

 

Pyridinium holds the key

 

The crucial element that provides CLEARFIL™ SE Protect with its antibacterial cavity cleansing ability is contained in the MDPB monomer. The pyridinium group in this monomer is positively charged, whereas the cell structures of bacteria are, generally speaking, negatively charged. This means that they are automatically attracted to the positively charged contact points of the MDPB monomer, causing these bacteria cell membranes to lose their electrical balance and be destroyed. This process—bacteriolysis—kills the bacteria.

 

The bactericidal mechanism of MDPB is presumed to be similar to that of the well-known antibacterial agent cetylpyridinium chloride, which is in many toothpastes and mouthwashes.

 

Certified biocompatibility

 

The MDPB monomer is immobilised during the polymerisation of the bonding and remains latently active without affecting the dental patient’s own cells. Unlike the situation when other cavity hygiene products are used, this process—patented by Kuraray Noritake Dental—does not have a negative impact on the bond strength and even ensures that there are no long-term cytotoxic effects on the patient’s own body cells such as odontoblasts. These results have been confirmed by the official biological evaluation for medical devices known as the ISO 10993 series.

PRESENTATION OF A STUDY WITH ILLUSTRATIVE CASES

INTRODUCTION

 

In recent years, the application and advancement of digital technology in dentistry has made it possible to accomplish the fabrication of highly accurate prosthetic zirconia appliances that were difficult to mill using the previously available technology. In addition, thanks to advances in adhesive dentistry and the advent of cements that bond strongly to a diverse range of materials, cements have come into wide clinical use that can cope with the many types of materials used for the fabrication of prosthetic appliances.

 

At our hospital, we select the treatment method most suitable for each case by appropriately specifying various types of prosthetic appliance according to the status of each case. For example, we may specify zirconia prosthetic restorations fabricated by the CAD/CAM system, or silica-based ceramic prostheses, or those made of lithium disilicate glass, as the case dictates.

 

PANAVIA™ V5 is a resin cement system that bonds strongly to various types of prosthetic appliance, as well as to tooth structure. PANAVIA™ Veneer LC, a new resin cement system developed by Kuraray Noritake Dental Inc., has suitable characteristics for bonding laminate veneers, using two types of primer that can be used in common with PANAVIA™ V5. Here are some clinical examples of its advantages as a resin cement system used for laminate veneers restorations.

Applied and rubbed for more than 10 seconds. Then, it
was washed off sufficiently (until the cleaner color had
completely disappeared), and dried with compressed air.

left lateral incisors have been improved, providing a good

Fig. 1 The patient was referred by an orthodontist. The main
complaints were improper aesthetics of the teeth due to dark
triangles betwen the teeth and incisal wear.

Fig. 2 On the basis of the pre-treatment diagnosis using
a mockup, the teeth were prepared, with keeping in mind
that the enamel should be preserved to the maximal extent
possible.

Fig. 3 A fixation retainer was present at the palatal side,
making it difficult to take coventional silicon impressions.
Therefore, an intraoral scanner was used.

Zirconia STML laminate veneer to complete the restorations.
The inner surface of each restoration was sandblasted, being
careful to prevent chipping.

Versatile flowable composite for everyday dental challenges

AWARD WINNING PRODUCT

For the 9th year in a row, CLEARFIL MAJESTY™ ES Flow (Low) has been named a Top Product by Dental Advisor, which announced in its JAN/FEB 2024 issue its choices for dental materials and equipment found to deliver the best practice-based performance based on the publication’s annual review of such products.

 

ABOUT THE PRODUCT

CLEARFIL MAJESTY™ ES Flow, which is available in three different levels of flowability, was the Top Product honoree in the Direct Restorative Category and was also given Editor’s Choice award for its medium viscosity version - Low.

 

Rather than limit practitioners to a single viscosity that might not be ideal for all flowable composite indications, CLEARFIL MAJESTY™ ES Flow comes in one version firm enough to stay in place when applied, another malleable enough to fashion a specific shape, and a third runny enough to flow into every corner or undercut. The practitioner would thus be able to select the best version for the job at hand—choosing the high flowability variant for cavity linings and as a resin coat in immediate dentin sealing (IDS) procedures, but instead, the super low flowability option for direct veneer procedures and for the build-up of cusps, etc.

Beyond its handling advantages, this universal flowable composite, which incorporates Kuraray Noritake Dental Inc.’s Light Diffusion Technology, is highly esthetic, with optical properties that enable the practitioner to fashion lifelike restorations that integrate seamlessly with the surrounding tooth structure.

 

EVALUATION

The flowable composite tested by the 29 DENTAL ADVISOR consultants in 909 uses in their dental practices was the medium flowability variant CLEARFIL MAJESTY™ ES Flow (Low). Their evaluations focused on properties including placement/handling, esthetics, viscosity, and polishability, and they rated all these key features to be “excellent”. This led to a recommendation rate of 100% and a 98% overall clinical rating.

 

REMARKS FROM EVALUATORS

Among their comments were: “The material blended so well with the tooth structure that you had to really look to find the interface.”. “Readily apparent on radiographs.” and “One of the best flowable composites I have used.”.
